Herein this Agreement is between the above named and below signed Broadcaster representing the above named Broadcasting Company (herein called the “Broadcaster”) and the above named and below signed Artist(s) and Management/Agent (herein called the “Artist”), for the above named Project/Promotion (herein called the “Project”) to be broadcast on the above named Program (herein called the “Program”).

Herein the term Broadcast includes Radio, Television, Internet and Satellite transmissions and/or any other means of transmission to the viewing and/or listening public.

HENCE, in consideration of the promises, and of the mutual undertakings herein contained, and for other good and valuable considerations, the above named and below signed Artist and the above named and below signed Broadcaster agree as follows:

1. The Artist hereby agrees to perform for the Broadcaster for the purposes of recording a Performance. The Artist hereby grants permission to record Artist's Performance for the sole purpose of re-broadcast at the dates and location detailed in Appendix “A” attached hereto.

2.The Artist may choose the material suitable for airplay by notifying the Program within _______________________________ days of the date of performance. It is the sole discretion of the Broadcaster if any of the material is suitable or unsuitable for airplay. . . continued
